We have been informed of a data breach of client information from the servers that our web-based reception software (
www.practicehub.io) utilises. Unfortunately, your client record was involved in this intrusion. We have been waiting to receive more detailed information before we contacted you.
These documents have been deleted, possibly stolen. In this instance the documents were composed of scans of your New Patient Form, the History of your complaint for primarily musculoskeletal issues, our Examination and some treatment pages imported from the previous software used as well as original paper documents.
Any paperwork completed online has not been affected, this includes any ongoing treatment notes and paperwork uploaded from last Thursday.
The Information Commissioners Office (
https://ico.org.uk/) has been informed of the breach and we understand that Practice Hub is not the only business on the server affected.
As a result of this issue please be aware of any phishing (unsolicited emails or phone calls) attempts you may receive. However, as the documents were all scanned, and not a digital record, it will be harder to retrieve the information from them.
The only communications you will receive from Worksop Chiropractic will be pertinent to your appointments or related to a clinic administrative procedure. We do not keep a record of any of your financial details (i.e., bank account / card details).
The security of your client record and our files is of the utmost importance and we adhere to GDPR procedures. Practice Hub have informed us of their improved data protection procedures moving forward to prevent this occurring again.
